Dementia Home Treatment Team

The Dementia Home Treatment Team provide a hospital at home approach to prevent people living with dementia from having to go into hospital when their needs are escalating.

The team provide short-term additional, intensive support in their own home or place they live, with as much autonomy and independence as possible.

The countywide team help by:

  • Caring for people at home
  • Offering short term intensive support
  • Bringing different professions together to help
  • Avoiding unnecessary hospital admission
  • Support, information and signposting for carers
  • Providing detailed assessment and support when people's needs are escalating
  • Develop treatment plans for people with complex needs, working with other agencies as required.
  • Can support with hospital admission where this still may be required and can facilitate earlier discharge.

Who do the team support

The team will support people living with dementia of any age who are at risk of admission if additional support is not provided

Who will I see?

The Dementia Home Treatment Service (DHTT) is a multi-disciplinary team, consisting of:

  • A Clinical Team Leader / Independent Nurse Prescriber
  • Nursing staff
  • Occupational Therapist
  • Assistant Practitioners
  • Specialist Doctor

Who you will see will depend on what your individual needs are

How do I get referred?

Referrals are made via the Trust's Community Mental Health Teams where they assess there is a need for escalation.

Any referrals will be responded to within 12 hours and once accepted, the team will make contact with you or carer/home to book an assessment appointment.

Operating hours

The team operate 7 days-a-week from 8am to 8pm.

Out of Hours emergency information will be given on the initial assessment from the team. This includes what to do if the service user requires urgent support.

The team operate a duty worker system 8am to 8pm Monday to Friday and

10am to 6pm Saturday and Sunday

 

There is an answerphone for out-of-hours calls but these will not be responded to until the following day
